Microtia ear reconstruction

This ear reconstruction video shows a surgery to correct microtia by Toronto Plastic surgeon, Leila Kasrai MD. Microtia means small ear, and it is a congenital abnormality which causes problems with the formation of the ear tissue. In some children, a small portion of tissue is seen where the ear would normally be found. In others, the ear lobe or hollow of the ear, may be partially formed with the rest of the ear tissue missing. The most severe form of this condition leaves almost no ear tissue visible.

chondrodermatitis Nodularis Helicis

This picture shows the ear of a 74 year old female who has a one year history of a slightly painful lesion on her auricle. The lesion was characteristic of chrondrodermatitis nodularis helicis (CDNH).
CDNH is a slow growing painful lesion which may reach a size of 2 to 4 mm. It begins as a nodule over the cartilage folds of the ear and may progress to form a central crust and ulceration. The cause is unknown but CDNH usually occurs on the side the patient sleeps and may be related to trauma. The most common location is on the mid portion of the antihelix.

Treatment is to excise the lesion and underlying inflamed cartilage. Steroid injections have also been advocated but in my experience they have not been successful.

An osteochondroma or exostosis is a bony projection of the surface of a bone capped by cartilage.

It is the second most common benign primary bone tumour after non-ossifying fibromata.

This condition affects males and females equally in their first decade, and usually found in the lower femur, upper tibia and upper humerus i.e. the metaphysis of long bone.

The condition can be single or multiple, the later is associated with a 20% rate of malignancy, compared to less than 1% in the presence of a solitary tumour.

Button battery in ear; How dangerous is it ?

Young children commonly place foreign bodies in the ear canal and nose. If a vegetable foreign body is present, do not use ear drops because they will cause the foreign body to swell.

One of the most dangerous foreign bodies is a button battery. This type of foreign body should be removed immediately. Prior to removal, the use of ear drops of any kind is absolutely contraindicated.

The electrical charge of the battery will produce electrolysis of any electrolyte-rich fluid. This produces hydroxides which will cause a severe alkaline burn. The picture above shows a severe burn of the external auditory canal which resulted in the exposure of bone over 180 degrees of the ear canal's circumference. The battery was in the child's ear for approximately three days and the picture was taken one month after the battery was removed. Healing is slow and damage to the eardrum, facial nerve, and inner ear can occur. Luckily in this patient the injury was confined to the ear canal.

Sulcus Vocalis

Sulcus vocalis is a defect or divot in the surface of the vocal cord. This can be congenital or acquired. Vibration of the vocal cord is impaired at this location, leading to a poor traveling waveform and hoarseness.

Cricopharyngeal spasm

Cricopharyngeal spasm is dysfunction of the upper most valve of the esophagus. This muscular valve normally allows the esophagus to open during swallowing. In cricopharyngeal spasm, this valve does not open properly, causing difficulty swallowing and resulting in a significant collection of liquid or food in the back of the throat.

Tracheal stenosis is a narrowing or scarring of the inside of the trachea can produce difficulty breathing;
treatment may be surgical excision, dilation, or stenting.

Septal Hematoma

a septal hematoma is a collection of blood between the nasal mucosa and the cartilaginous septum.Typically soft and fluctuant when compressed, It may occur unilaterally or, more commonly, bilaterally following nasal trauma with or without associated nasal fracture. This urgent condition warrants immediate referral to an otolaryngologist.

Management of septal hematoma includes local anesthesia with needle aspiration or incision and drainage. Several days of nasal packing prevents reaccumulation of blood. Ideally, treatment occurs within a few hours of injury. If the hematoma is not cleared, the septal cartilage may become necrotic and result in a saddle nose deformity or septal perforation. Accumulated blood may become infected, and antibiotics should follow drainage if abscess is noted.
